What are All-on-4 dental implants?
All-on-4 dental implants replace every tooth in one jaw with a single fixed arch of teeth held on four titanium implants. The implants do the job the tooth roots used to do, the arch does the job of the teeth, and the whole thing stays in your mouth permanently — you do not take it out to clean it and you do not take it out at night. It is a treatment for people who have already lost most of an upper or lower jaw of teeth, and for people who have been told the teeth they still have cannot be saved.
It is also worth being clear about what it is not. All-on-4 is not an improved denture, and it is not a small procedure: it is oral surgery, it carries the risks any surgery carries, and the arch needs real cleaning every day for the rest of its life. What it offers in return is a set of teeth that do not move.
Why four implants is enough
Four implants can carry a full arch because of where they are put, not because the arch has been made to do less. The two at the front go in upright, into the deeper, denser bone at the front of the jaw that tends to survive longest after teeth are lost. The two at the back are tilted forwards, so that their length lies in that same solid bone rather than reaching back towards the sinus in the upper jaw or the nerve canal in the lower one.
Tilting has a second effect that matters just as much: it brings the back implants out further along the arch, widening the footprint the bridge stands on and shortening the unsupported stretch behind the last implant. Four well-spread anchor points are steadier than four bunched at the front. That geometry is also why so many people told they need a bone graft can have full mouth dental implants without one — the implants are routed into bone that is already there rather than into bone that has to be built first. Where a graft or a sinus lift is still needed, it is covered by the price rather than added to it, so the scan finding one does not change what you pay.
What the fixed arch is made of
The arch is one piece — teeth and gum together — screwed down onto the implants through small access holes that are then filled in, so nothing shows. You will have two of them over the course of treatment. The temporary arch is fitted on the day of surgery and is usually a hard-wearing acrylic, built to look right and to let you talk normally straight away rather than to stand up to heavy chewing — which is why the food advice during healing matters.
The final arch is made once the implants have integrated and is a more substantial piece of work: commonly acrylic teeth on a metal framework, or a solid zirconia arch, chosen with you according to your bite and whether you grind your teeth. Because it is screwed rather than cemented, it can be unscrewed years later so everything underneath can be cleaned and inspected, then refitted. That access is a real long-term advantage, and the reason maintenance appointments are worth keeping.
How All-on-4 differs from full arch implants placed one at a time
Conventional full arch implants rebuild the jaw in sections, on six, eight or more implants carrying separate bridges or crowns. Where the bone at the back has shrunk, those back implants usually need grafting or a sinus lift first, which adds months of waiting and a second surgical procedure. All-on-4 sidesteps most of that by angling the back implants, and compresses the surgical side of treatment into one appointment that ends with teeth in a day — implants placed and a fixed temporary arch attached before you go home.
The trade-off is worth knowing before you choose. With individual full arch implants a single crown can be replaced on its own; with All-on-4 the arch is one unit, so a chip or a repair usually means the whole arch comes off, goes to the laboratory and comes back. Where there is plenty of bone, or where you would rather have teeth restored in separate sections, conventional implant treatment can still be the better answer. Your CT scan and your priorities decide it, and we will say so if All-on-4 is not the right route for you.

3D CT scan and digital planning

A CT scan shows exactly how much bone you have and where it is. We plan the position and angle of each implant digitally before you come in for surgery, so nothing is decided on the day.

Any failing teeth on the arch being treated are removed at the start of the surgical appointment, under local anaesthetic.
